Register of Deeds - Passports

PASSPORT APPLICATIONS NEED TO BE COMPLETED IN BLACK INK
 

PASSPORT FEES TO THE DEPARTMENT OF STATE MUST BE CHECK OR MONEY ORDER

The Register of Deeds office accepts passport applications Monday through Friday by appointment only:  9 AM to 11 AM and 1 PM to 3:30 PM.  Please call 906-635-6312 to arrange for appointment.  No appointments will be made online.

Most applications are currently requiring 6 to 8 weeks for normal processing.  Expedited service is available at an additional cost.

 THE FOLLOWING DOCUMENTS ARE REQUIRED WHEN APPLYING FOR A U.S. PASSPORT BOOK OR CARD:

  • Proof of citizenship: Certified copy of birth certificate or naturalization certificate.

  • Picture identification: Such as valid driver's license, military identification or passport.

  • One Passport quality photograph. 2"x2" in size with a white background.

  • PASSPORT PHOTOGRAPHY SERVICE IS NOT PROVIDED BY THE REGISTER OF DEEDS OFFICE. PLEASE BRING PASSPORT PHOTOS WHEN APPLYING FOR PASSPORT AT REGISTER OF DEEDS OFFICE.

PASSPORT CARD:

 

Adult:  $30.00 check/money order payable to: The Department of State

              $35.00 check/money order/cash payable to: Chippewa County

 

Child: (Age 15 & under)

                 $15.00 check/money order payable to: The Department of State

                $35.00 check/money order/cash payable to: Chippewa County

BOTH PASSPORT BOOK & PASSPORT CARD:

 

Adult:  $160.00 check/money order payable to: The Department of State

Expedite Fee:  $60.00 added to the fee made payable to the Department of State. Expedited service is currently running between 5-7 weeks.

PLEASE NOTE:  The passport card may only be used for international travel by land or sea between the United States, Canada, Mexico, Caribbean, and Bermuda.  THE CARD MAY NOT BE USED TO TRAVEL BY AIR. 

If you need to inquire about the issuance of your passport

The National Passport Information Center (NPIC) can assist you.  You may speak with a Customer Service Representative by calling 1-877-487-2778.  **TTY/TDD Users: 1-888-874-7793.

Customer Service Representatives are available from 8:00a.m. to 8:00p.m. EST, Monday-Friday (Excluding Federal Holidays).

 

For passport and other travel information:  Visit the U.S. Department of State's Bureau of Consular Affairs, which includes Passport Services, on the internet at www.travel.state.gov OR dial 1-202-647-5225 to hear recorded information and travel warnings.  For auto-fax infromation, dial 1-202-647-3000. 

IF YOU WERE BORN OUTSIDE THE UNITED STATES TO AT LEAST ONE U.S. PARENT


Submit a Consular Report of Birth (Form FS-240), Certification of Birth (Form DS-1350 or FS-545), OR

  • Your foreign birth certificate.  Canadian-born individuals must submit the long form of their birth certificate with a raised seal.  Individuals who do not have a long-form birth certificate in their possession may go on-line to serviceontario.ca birth certificate, and follow the prompts to apply for a certified copy of the record.

  • Your U.S.-born parents’ birth certificates.  Certified copies (with raised seal)  from the county in which they were born;

  • Your parents’ marriage license.  Certified copy (with raised seal) from the county in which they applied to be married;

  • An affidavit stating all of your U.S. parent's periods and places of residence/physical presence in the U.S. and abroad before YOUR birth with signatures NOTARIZED. 

Submit the above documents along with the proper fees to the Chippewa County Register of Deeds.  You must also complete the DS-11 Passport application form and submit proper passport photos.
